The game takes place in an alternative future, in the fictional country of Lasia, Russia´s satellite country. In Soviet times, an outbreak of a dangerous virus occurred in Lasia. Subsequently, the infection was suppressed by the troops, the samples of the virus and some of the creatures needed for the experiments were closed in a special laboratory.

After some time in Lasia, a new infection begins, what is responsible for the former CIA agent Mezner, who became a traitor for mercenary purposes. He is trying to eliminate the main character, a member of the US Army Special Forces, Hayden Tenno (English Hayden Tenno). During the operation, he himself becomes infected with a virus. This leads to the fact that his hand is covered with metal, and its continuation becomes glaive, a special throwing weapon. The protagonist makes his way through the crowds of mutants and soldiers to eliminate the danger.
